The short answer — Enabling services — case management, transportation, translation/interpretation, health education, outreach, and eligibility assistance — are mandatory under the Section 330 Health Center Program requirements and are reportable on UDS Table 5. Yet most FQHCs either absorb the full cost as grant overhead or leave billable Medicaid encounters uncoded. In 2026, 39 states allow some form of Medicaid reimbursement for at least one enabling service category, and CMS has clarified that FQHC cost reports may include enabling service personnel costs when the services are clinically integrated. The revenue gap is real: a clinic with 4,000 Medicaid patients actively using transportation or CHW case management can recover $50K-$180K annually by building a proper enabling-service billing workflow — UDS-aligned, cost-report-defensible, and coordinated with the sliding fee discount policy.
